So I switched to reCaptcha for 1 day and we got about 20 spam accounts and 3 spam posts.
Back to Q&A as it seems to actually work.

The most effective way is probably to add some custom validation, like just an extra textbox and the question "are you a bot (type no)?". Then only allow people writing no.
No one is going to bother targetting this forum specifically.
